• Partner with members and families to identify needed supports and services for personal health and independent living goals
• Collaborate with clinical and social support teams to address members' physical and behavioral health needs
• Conduct thorough face-to-face assessments to evaluate members' situations and needs
• Authorize and confirm home and community-based services for members
• Maintain compliance with driving and safety requirements for fieldwork

• Minimum of 3 years of experience in a clinical setting
• Active Registered Nurse license in Tennessee or a valid license in a participating Nurse Licensure Compact state; or a Master’s degree in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW)
• Must reside within Sumner or Robertson counties
• Effective time management and strong interpersonal skills
• Proficient in basic Microsoft Office and email

• 2 years of clinical experience preferred
• Exceptional customer service skills
• Strong oral and written communication abilities
• Previous experience in community health or social services
• Familiarity with health and wellness programs
