Responsibilities
  • Deliver high quality health care within Fast Pace Health’s scope of services while achieving optimum patient satisfaction.
  • Provide preventative care as the patient’s first point of contact and maintain ongoing, comprehensive care focused on long-term health and well-being.
  • Work in a team-oriented environment; adapt to new technologies and protocols in a fast-evolving practice setting; respond quickly and accurately to changes in condition or response to treatment.
  • Provide outstanding patient service within the clinic and through various virtual channels while maintaining a compassionate and welcoming atmosphere.
  • Conduct thorough physical examinations and assessments for patients of all ages (children 3 years and up).
  • Perform chronic care management for conditions such as hypertension, diabetes, and asthma; administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
  • Order, interpret, and evaluate diagnostic tests to diagnose and assess patients’ clinical problems and needs; collaborate with physicians and other healthcare professionals to develop care plans.
  • Prescribe medications, recommend treatments, and provide written home-going instructions; refer patients to specialists or other resources as needed.
  • Educate patients and families on health maintenance, preventive care, health literacy, and shared decision-making; participate in quality improvement initiatives and value-based care programs.
  • Promote patient-centered care, continuity of care, and strong patient-provider relationships; adhere to core values of teamwork, communication, empowerment, quality of care, and friendliness.
  • Support compliance with health information management requirements, including HIPAA; complete mandatory compliance training.
  • Contribute ideas to improve patient experience and care; communicate regularly within the team to support excellent patient care.
  • Maintain accurate patient charts in a timely manner and manage documentation across in-person and telehealth settings.
  • Assist with urgent care activities as schedule allows; perform other duties as assigned by management.
Essential Functions
  • Provide care to meet patient volume goals (approximately +18 patients per day).
  • Preferred experience as a Telehealth clinician.
  • Lead clinic staff in behaviors and attitudes to deliver excellent patient care; guide team members to ensure quality services and satisfaction.
  • Discuss and review patient history, symptoms, allergies, and medications to form accurate diagnoses and guidance.
  • Engage with clinical leadership to ensure strong patient care and adherence to company values.
  • Ensure accurate completion of patient charts and forward charts as appropriate daily.
  • Work efficiently in a fast-paced autonomous environment; stay current with medical advancements and evidence-based medicine.
  • Implement clinical protocols as outlined by the CMO and Supervising Physicians; operate within standard NP protocols.
  • Analyze patient histories, symptoms, and diagnostic information to develop appropriate diagnoses; provide in-person and telehealth recommendations with patient-centric focus.
  • Ensure activities comply with department expectations and federal/state regulations, including HIPAA knowledge.
  • Complete mandatory compliance training and adhere to company core values.
