Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, in‑home nursing care and education for patients with life‑limiting illness. You will assess, implement, and adjust plans of care in coordination with an interdisciplinary team, ensuring comfort and dignity for patients and families.
You will educate caregivers, document care accurately, and participate in on‑call activities as part of a supportive hospice environment. A valid RN license and CPR certification are required.
