Role:
Hospice Registered Nurse Case Manager
Responsibilities- Provide compassionate end-of-life care in patients’ homes
- Coordinate and deliver skilled nursing care
- Manage pain and symptoms effectively
- Develop individualized care plans in collaboration with an interdisciplinary team
- Educate and support patients and families
- Ensure timely and accurate documentation
- Communicate closely with physicians and caregivers
- Registered Nurse licensure
- Strong communication and interpersonal skills
- Ability to work independently with autonomy
- Commitment to providing compassionate care
- Willingness to engage in ongoing clinical training
