Social Service Assistant
Full-Time | Part-Time | Day Shift
We are seeking a compassionate and organized Social Service Assistant to support the psychosocial, emotional, and social needs of residents. This role works closely with the Social Services Director and interdisciplinary team to promote resident dignity, independence, and quality of life while ensuring compliance with federal and state regulations.
This position is well suited for someone who is resident-focused, detail-oriented, and comfortable working in a long-term care environment.
Responsibilities- Assist in identifying and addressing residents’ psychosocial, emotional, and mental health needs
- Complete social histories, psychosocial assessments, and required documentation
- Participate in interdisciplinary care planning and resident care conferences
- Support resident rights, advocacy, and grievance resolution
- Assist with PASRR processes, advance directives, and resident rights education
- Provide support to residents and families during admissions, room changes, and transitions
- Coordinate discharge planning activities and community resource referrals
- Assist with Medicaid applications and benefit coordination as directed
- Document progress notes and resident interactions accurately and timely
- Support surveys, audits, and regulatory compliance efforts
- Participate in quality assurance and performance improvement activities
- Maintain confidentiality and comply with HIPAA requirements
- High school diploma required
- Prior experience in social services or long-term care preferred
- Knowledge of resident rights and psychosocial needs of the elderly preferred
- Strong communication and documentation skills
- Compassionate, professional, and team-oriented
- Ability to manage sensitive situations with discretion
- Standing, walking, and moving throughout the facility
- Sitting for documentation and meetings
- Exposure to resident care environments
