Position: Registered Nurse (Per Diem)

Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.

Responsibilities:
  • Assess and evaluate patient’s status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
  • Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required.
  • Provide and maintain a safe environment for the patient.
  • Attend interdisciplinary group meetings.
Additional Duties:
  • Participate in on-call duties.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfill the obligation of requested and/or accepted case assignments.
  • Actively participate in quality assessment performance improvement teams and activities.
Qualifications:
  • 2+ years’ nursing experience, preferably in geriatrics is desirable.
  • Degree in Nursing from an accredited institution and an active nursing license in Virginia.
  • Proficient with electronic medical records.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Possess a valid driver’s license and own a reliable, insured vehicle.
