Overview
The purpose of this position is to help people access waiver services based on their age, income, disabilities, and risk for institutional care. This position is also responsible for completing MnCHOICES assessments to determine eligibility. For those who are eligible, the Case Manager coordinates daily living, health-related, community, and vocational services based on the individual’s assessed needs and preferences.
Responsibilities- Conduct comprehensive initial and reassessments to determine client needs and level of care according to MN Department of Human Services statutes for HCBS program eligibility. Makes determinations for R185/DD Eligibility under MN State Statute guidelines. (Daily – 20%)
- Provide individuals and families with recommendations and options for services. Develop person‑centered plans of care based on assessed needs. Attend care conferences, team meetings, and intermittent client visits for updates. Ensure providers deliver authorized services. (Daily – 20%)
- Prepare and maintain client records that meet federal, state, and local guidelines, including documentation of case management services, assessment activities, and correspondence with providers; ensure compliance with applicable protocols. (Daily – 15%)
- Interpret agency, health plan, and Department of Human Services policies for clients and families. Participate in professional development activities, including trainings, webinars, and regional meetings. (Daily – 5%)
- Provide ongoing case management to clients per the CBSM (Community Based Services Manual) and other DHS policy for HCBS programs and MCOs. Conduct face‑to‑face visits with clients/families/guardians at least twice a year. (Daily – 40%)
- Maintain accounting of time using the Social Services Information System (SSIS) and perform other administrative tasks, including staff meetings, staff training, and clinical supervision. (Daily – 5%)
- Demonstrate effective written and verbal communication with clients, the public, and staff. Maintain good working relationships with clients and the public. Use effective strategies to assist clients with limited English proficiency. (Daily)
- Perform other duties of a similar nature or level. (As required)
- Valid Minnesota Driver’s License.
- A bachelor’s degree in social work, nursing, or a closely related field.
- Two (2) years of home and community‑based experience in assessment and care plan development; or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
- At least one (1) year of experience in education or treatment of people with developmental disabilities or related conditions.
