Base pay range$70,000.00/yr - $85,000.00/yr
Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking an ICM Licensed Medical Social Worker.
Primary Responsibilities- The CM Licensed Medical Social Worker provides social services to members with challenging medical and behavioral health issues.
- Receives referrals for members who screen positive for depression and are in need of additional BH support.
- Responsible for supporting post-discharge follow up for referred members discharged from the hospital (inpatient or ED) with active BH needs.
- Fosters the interaction between members and medical or behavioral health (BH) healthcare providers and serves as liaison to the behavioral health ASO to coordinate medical and behavioral health care.
- Collaborates with the behavioral health ASO to coordinate monthly co-management meetings and assure optimal clinical outcomes for co-managed members.
- Maintains a caseload, monitoring patient progress with effective treatment, and facilitating appropriate medical and BH follow-up as necessary.
- Collaborates with CM case managers to comprehensively care plan for members with medical/BH co-morbidity. Provides education and support to Care Management staff on handling behavioral concerns and crisis situations.
- Participates in care coordination and community collaborative meetings as needed.
- Completes BH assessment with the member telephonically or via video conference/face-to-face visit.
- Conducts depression screening and refers members who screen positive to appropriate resources.
- Provides case management services in conjunction with other CM staff and community providers in emergency and non-emergency situations.
- Documents member and provider interactions as required and maintains records of referral interactions with behavioral health ASO.
- Directs enrollees to appropriate community resources.
- Provides psychosocial support across CM teams.
- Maintains a monthly caseload and accepts consultations with staff as needed.
- Identifies and refers members eligible for waiver and community-based programs; assists in the application process when needed; and tracks application process for future care coordination.
- Participates in quarterly peer review/audits as needed.
- Demonstrates strong organizational, time management skills and application of appropriate critical thinking capability.
- Establishes relationships with, and acts as liaison with BH providers, social service agencies and community resources.
- Keeps staff updated on available resources throughout the state.
- Provides assistance to staff and providers in the evaluation, diagnosis appropriate treatment and referral of members referred for medical and behavioral health.
- Works in collaboration with other healthcare providers/caregivers to evaluate the member’s medical, physical or behavioral health condition.
- Communicates with CHNCT members establish and maintain relationships.
- Provides care management in conjunction with other management staff and community providers.
- Demonstrates experience in crisis intervention.
- Complies with all organization policies and regulatory requirements related to case management.

Other QualificationsDirect experience in handling caseload with chronic medical and behavioral health issues. Familiar with health care services delivery to economically disadvantaged populations and medically underserved.
