Job Summary

Under the clinical direction of the Nurse Case Manager and the administrative direction of the Aide Supervisor
, the CNA/HHA provides hands‑on care to adult clients in their homes. This role focuses on personal care, safety, dignity, and comfort – while maintaining accurate documentation in accordance with regulatory standards.

What You'll Do
  • Provide direct, one‑on‑one care to clients in their homes
  • Assist with personal care such as bathing, grooming, dressing, and toileting
  • Safely lift, transfer, and assist clients with mobility
  • Observe and report changes in client condition to the care team
  • Document care activities accurately and on time
  • Work flexible hours to support client needs (great for per‑diem caregivers!)
What We're Looking For Education
  • High School Diploma or GED required
Experience
  • 1+ year of healthcare or caregiving experience preferred
Certification
  • Home Health Aide (HHA):
  • Employed as a nurse's aide within the past 24 months OR
  • Willing to pass an agency-approved competency test
  • Certified Nurse Aide (CNA):
  • Active Connecticut Nurse Aide Registration required
