At B1C, immigrants acquire the tools they need to thrive. We provide free services that educate, employ, empower and engage immigrants and the broader community. Our programs include English Language classes, Family and Individual Support Services, Academic Enrichment Programs, Immigration Legal Services, and Workforce Development programs.
PositionThe Community Support Coordinator will connect immigrant individuals and families to health resources and support services, with a primary focus on the Healthy Steps program, health navigation, health education, and the Mental Health Initiative. This role also provides general case management and family support as needed, helping participants access the broader range of B1C programs and community resources. This part-time, 25-hour-per-week position requires on-site attendance and reports to the Family Services Manager.
It involves occasional evening and weekend assignments.
- Conduct culturally sensitive health and basic needs assessments to identify gaps in access to medical care, nutrition, insurance, mental health, and other related areas.
- Develop personalized care or resource plans in coordination with each participant's health goals and social needs.
- Coordinate and deliver the Healthy Steps program and support the Mental Health Initiative, connecting participants to health and mental health resources and services, helping reduce barriers to access.
- Deliver individual and group health education, including preventive care, nutrition, and chronic disease management, using materials that are culturally and linguistically appropriate.
- Support participants in understanding and navigating insurance coverage, billing, and financial assistance.
- Connect program beneficiaries to both internal and external services to address critical needs and ensure access to the appropriate resources and support, and conduct participant follow-up to track progress and outcomes.
- Establish and maintain up-to-date case files in a timely manner; enter and manage data in the Salesforce database; generate reports, track trends, and analyze program outcomes to enhance and improve service delivery.
- Work hand in hand with fellow Family Services team members on events, donation distributions, and other shared initiatives, while also collaborating with the Community Engagement team to establish and expand connections with healthcare providers, clinics, and other health-focused community partners.
- Coordinate and support onsite health events in partnership with local providers (e.g., flu clinics, health day events, wellness workshops).
- Supervise and engage volunteers supporting health-related programs and events, ensuring they are effectively contributing to program goals and assisting beneficiaries with their needs.
- Fluency in Spanish and English with demonstrated written and verbal communication skills.
- Minimum of 2 years of experience in community health or public health with immigrant or underserved populations.
- Bachelor's degree in Public Health, Community Health, or a related field required.
- Certified Community Health Worker (CCHW) and/or Certified Application Counselor (CAC) preferred.
- Strong multicultural awareness, trauma-informed interpersonal skills, and compassion in working with diverse populations.
- Strong ability to prioritize and complete tasks efficiently, ensuring deadlines are met and projects are delivered on time.
- Flexibility to travel to satellite offices and other off-site locations as needed.
- Strong proficiency with Microsoft Office software and familiarity with Salesforce or other database systems.
- Belief in the organization's mission and goals, with strong commitment to the empowerment of low-income immigrant populations.
