Part-time opportunity available on Tuesdays and Fridays, with one Saturday per month.
The Primary Care Nurse (PCN) provides high-quality, comprehensive, and professional nursing care to individuals and families across the lifespan. As part of a clinical team, the PCN works at the top of their licensure to enhance safety, effectiveness, and efficiency of care through professional nursing responsibilities and delegation of technical and clerical functions to medical assistants as appropriate. The PCN offers health education on preventive, chronic, and episodic health issues.
They collaborate with primary care providers to coordinate care, support transitions, and educate patients to improve health outcomes. The PCN applies principles of advanced access, planned care, the chronic care model, and utilizes rapid cycle performance improvement using the PDSA approach at CHCI.
Responsibilities:
Support primary care of patients under the planned care model, collaborating with PCPs to identify needs for preventive care, evidence-based chronic disease management, and care coordination. Provide health education and assist patients in identifying and monitoring self-management goals. Coordinate care with community providers and follow up on ER visits and hospital discharges. Assist providers with exams, treatments, and procedures as needed.Ensure the safe, accurate, and appropriate collection, storage, and processing of biologic specimens. Administer medications, vaccines, and immunizations via appropriate routes under standing orders and provider directives. Monitor patient responses and educate patients about medications.Perform various nursing activities such as wound care and catheterization after initial prescription by a provider. Document all patient care accurately and timely. Schedule tests and referrals for specialty care as ordered.Maintain strict infection control standards according to CHCI protocols and OSHA/CLIA standards.Serve as a resource for medical assistants regarding patient preparation and procedures.Monitor and ensure follow-up of abnormal test results, including Pap smears, STDs, lead screens, etc.Oversee maintenance and ordering of supplies and equipment.Ensure compliance with mandatory annual training requirements.Utilize electronic health records proficiently to ensure timely, safe, and confidential patient information management.Education:
Bachelor of Science in Nursing (BSN)
