PRN Registered Nurse – Client Assessments & Care Oversight About Us

As we grow in Connecticut, we are seeking compassionate Registered Nurses to support client assessments, care planning, and caregiver guidance on a flexible, PRN basis.
Job Description for a RN Case ManagerThis role is ideal for an RN who enjoys supporting families, guiding caregivers, and building trust with clients, while maintaining a flexible and predictable workload. You will play an important role in ensuring our clients receive safe, high-quality, and person-centered care, without the demands of shift nursing or full caseload management.
Responsibilities of a RN Case Manager- Complete initial client assessments and periodic reassessments
- Develop and update individualized care plans tailored to client needs, preferences, and safety
- Conduct supervisory visits to support caregivers and ensure high-quality care
- Provide coaching, education, and encouragement to caregivers
- Communicate with families to offer updates, guidance, and reassurance
- Monitor client progress and recommend care adjustments when needed
- Document assessments, care plans, and visit notes in the agency's system
- Participate in occasional care conferences or team discussions to support continuous improvement
- Support compliance with Connecticut home care standards and agency policies
- Coordinate and oversee the implementation of care plans, including coordinating with other healthcare professionals and community resources as needed.
- Provide ongoing monitoring and evaluation of client progress and adjust care plans as necessary.
- PRN / As-needed
- Minimum commitment: 8 hours per month to start.
- Typical work includes client assessments and care plan reviews 1–2 times per month per client
- Scheduling is flexible and coordinated with your availability and agency needs
- No long-term shift or on-call requirements
Employment Type
- $50/day on-call + $125/hr. per assessment (1-hr minimum) + mileage reimbursement
- Paid for client assessments, supervisory visits, documentation, and approved clinical oversight time.
- Employment classification: W-2 PRN
- Active Registered Nurse license in Connecticut
- At least 2–3 years of nursing experience (home care, geriatrics, or case management preferred)
- Strong clinical assessment and documentation skills
- Warm, professional, and family-centered communication style
- Comfortable working independently in client homes
- Reliable transportation within Fairfield County, CT
