An internal medical group seeks an APRN or PA to provide care in skilled nursing facilities.

• Provide medical care to a complex patient population in short‑term rehab and long‑term care settings.
• Analyze patients’ medical history, medication allergies, physical condition, and exam to determine best care for acute and chronic medical conditions.
• Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical problems and health care needs.
• Prescribe pharmaceuticals and treatment regimens as appropriate for assessed conditions.
• Record findings and plans in the medical record within the required time frame.
• Coordinate with the facility care‑planning team to develop comprehensive care plans.
• Communicate with patients and families regarding admission, care plans, discharge needs, prognosis, and end‑of‑life care.
• Follow established departmental policies, procedures, objectives, quality improvement goals, and safety, environmental, and infection control standards.

• Master’s or doctorate prepared.
• Certified nationally as a Nurse Practitioner through AANP or ANCC.
• Valid Connecticut RN license and advanced‑practice RN license, and certification as a Nurse Practitioner.
• DEA and Connecticut controlled‑substance license.
• Experience working with elderly patient populations.
• Experience with electronic health records.
