An internal medical group seeks an APRN or PA to provide care in skilled nursing facilities.
Key Responsibilities- Provide medical care to a complex patient population in short‑term rehab and long‑term care settings.
- Analyze patients’ medical history, medication allergies, physical condition, and exam to determine best care for acute and chronic medical conditions.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical problems and health care needs.
- Prescribe pharmaceuticals and treatment regimens as appropriate for assessed conditions.
- Record findings and plans in the medical record within the required time frame.
- Coordinate with the facility care‑planning team to develop comprehensive care plans.
- Communicate with patients and families regarding admission, care plans, discharge needs, prognosis, and end‑of‑life care.
- Follow established departmental policies, procedures, objectives, quality improvement goals, and safety, environmental, and infection control standards.
- Master’s or doctorate prepared.
- Certified nationally as a Nurse Practitioner through AANP or ANCC.
- Valid Connecticut RN license and advanced‑practice RN license, and certification as a Nurse Practitioner.
- DEA and Connecticut controlled‑substance license.
- Experience working with elderly patient populations.
- Experience with electronic health records.
- State license.
