Job Description

Stamford Health is seeking a part‑time LPN for our Urogyn & Recon Surg practice. The Licensed Practical Nurse (LPN) coordinates patient care following all established standards of practice within the State and Federal guidelines, provides staff mentorship, and serves as a resource to both staff and management in problem identification and resolution.

Schedule

Will rotate between Norwalk and Stamford.

Mon: 8am‑4:30pm, Tues: 1pm‑4:30pm, Wed: 8am‑4:30pm, Thurs: 8:30am‑12pm, Fri: 8am‑3pm

Major Accountabilities / Critical Responsibilities
  • Reviews schedules and organizes patient flow, greets patients, documents concerns, questions, etc.; accompanies patient to exam/procedure room and prepares patient for physical exam. Conducts initial patient interview and reviews medical profile, documenting all required charting in EMR or manual charting as appropriate.
  • Obtains and accurately documents patient vital signs, weight; reports any abnormalities to physician. Assists with specimen collection and testing as necessary.
  • Assists patients as needed (walking, transferring, dressing, etc.).
  • Provides minor treatment as directed by physician and assists physicians with minor procedures.
  • Provides education as indicated after evaluating the learning needs of the patient and/or family.
  • Schedules patient testing and pre‑certifies insurance as required.
  • Responds to telephone messages (patient, physician offices, etc.).
  • Checks for expired supplies and medication at end of month.
  • Stocks and maintains inventory in exam/procedure rooms.
  • Orders supplies as needed.
  • Ensures exam/procedure rooms & equipment are cleaned after each patient.
  • Prepares exam/procedure rooms for next patient & physician needs.
  • Notifies patients of test results per physician instruction and follow‑up.
  • Attends scheduled staff meetings, in‑service education opportunities, and participates in continuing education programs. Formulates and maintains effective working relationships with employees and other managers of the health care team.
  • Respects and protects patient rights to confidentiality and privacy, discloses information only for professional purposes, and displays a positive attitude when interacting with physician office staff, physicians, and fellow employees.
Qualifications / Requirements
  • Graduate of an accredited school of nursing.
  • Valid license as Licensed Practical Nurse in CT.
  • BLS Certification.
  • Excellent customer service and communication skills.
  • Knowledge of medical terminology.
  • Minimum of 2 years previous experience in a similar position preferred.
  • Must be able to read, write and speak English.
