MJHS is seeking a dedicated Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments, developing individualized plans of care and goals for hospice patients. Assessments may occur in homes, hospitals or facilities, with admission to the program following NHPCO guidelines.
Candidates should have an Associate degree or Diploma in Nursing (BSN preferred), 2+ years in relevant nursing settings, and an active NYS RN license plus NYS Driver’s license.
