Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non‑profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro‑communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job DescriptionHousing Case Managers are responsible for providing comprehensive case management services to individuals experiencing homelessness or at risk of homelessness. This role involves a combination of advocacy, direct support, and resource coordination to meet the varied needs of clients. Key responsibilities include conducting assessments, implementing harm reduction strategies, delivering trauma‑informed care, intervening in crises, and coordinating resources. The goal is to empower clients to attain self‑sufficiency by equipping them with essential life skills and support needed to achieve housing stability and secure permanent housing.
The pay range for this position is $22.00 - $25.00 per hour, depending on experience. Hours are 8:00AM to 4:30PM.
ResponsibilitiesHousing Case Manager Duties:
- Must be able to maintain a caseload between 30-35 clients
- Housing Case Managers must establish and execute an individualized housing plan, tenant screening and tenant assessments.
- Housing Case managers must engage with clients on a weekly basis.
- Successful interactions with clients and/or members of the care team must occur weekly, totaling four days of service per month, utilizing three different methods of communication. Examples include telephonic communication, electronic communication (text or email), or written correspondence.
- Housing Case Managers are required to identify a minimum of three independent living skills or goals in 100% of client care plans accompanied by actionable steps. Milestones will be considered complete once each living skill or goal has been successfully achieved.
- Clients must be engaged in person at least once a month.
- Notes must be entered into the Electronic Health Record (EHR) in the client's profile within 24 to 48 hours following any engagement or contact with the client.
- Housing Case Managers are responsible for connecting clients with local community resources and offering referrals to other agencies for additional housing opportunities.
- Housing case managers are responsible for assisting with unit applications and providing transportation for clients to property viewings.
- Attend Interdisciplinary Care Team meetings on a weekly basis to evaluate client goals and track the completion of action items. Monitor progress by utilizing the Electronic Health Record (EHR) and Homeless Management Information System (HMIS) databases to uphold accurate client information and program data.
- Link clients to resources to address psycho-social and daily needs such as health care, food, hygiene items.
- Perform crisis intervention, as necessary, and counsel client with confronting issues adversely affecting their lives, such as mental illness, addiction and abuse.
- Enroll families & individuals into the Homeless Management Information System through the Coordinated Entry System (CES)
- Housing Case Managers are required to attend training sessions to remain informed about housing laws, trauma‑informed care practices, and harm reduction strategies annually.
- Networking with landlords/property managers and other community entities.
- Conduct health and housing quality standards (if applicable) walk through of prospective units.
- Confirm the client’s move‑in costs and collect all supporting documents, including but not limited to the W‑9 form and the signed rental agreement.
- Understand fair housing laws, local housing laws and various housing programs within the community.
