- Conducts intake and preliminary interviews to determine needs of clients/families.
- Provides support to assigned group of individuals.
- Conducts counseling sessions as needed.
- Investigates allegations, provides advocacy, and assesses degree of risk to client.
- Develops and implements case plans for client based on needs.
- Provides for necessary arrangements according to client's needs; contacts other community services/agencies as necessary.
- Arranges transportation for clients' appointments.
- Testifies in court if necessary regarding clients' needs.
- Maintains accurate documentation and records on clients and their progress.
- Prepares required reports as mandated by federal, state, or County policies and procedures.
- Makes home visits to clients as necessary.
