Clinical Nurse Specialist, RN Nurse
The Role
We're looking for a skilled, compassionate Registered Nurse to step into a dual-function hybrid role at Abby Care — combining hands‑on patient case management with clinical education and caregiver training.
Based in State College, PA
, you'll serve as an RN Case Manager
, coordinating comprehensive care for a diverse patient caseload, while also taking on a Clinical Instructor function, preparing the next generation of Home Health Aides and contributing to our curriculum and training programs.
This is a rare opportunity for a nurse who wants to make an impact at both ends of the care continuum, delivering care and building the workforce that delivers it. You'll report into the Associate Director of Nursing and collaborate closely with clinical teams, PCPs, family caregivers, and HHA students.
Key ResponsibilitiesPatient Care and Coordination:
- Manage a diverse caseload with quality care and efficient coordination.
- Develop and implement individualized care plans based on assessments and goals.
- Coordinate with PCPs and internal teams for comprehensive care.
- Coach and oversee HHAs to ensure adherence to clinical standards.
- Educate patients and families on health needs and treatment plans.
Clinical Documentation:
- Maintain accurate and timely electronic charting, with a preference for real‑time documentation.
- Ensure all documentation meets regulatory and Abby Care standards, with a minimum of 24 hour turnaround time.
- Complete administrative tasks (e.g., mileage reimbursements) thoroughly.
HHA Training & Instruction:
- Design and implement skill training classes for HHA students.
- Support students during in‑person training at long‑term care facilities.
- Collaborate with clinical team to develop additional learning modules.
- Enhance students' clinical skills and medical knowledge through creative curriculum.
Quality & Communication:
- Build effective relationships with PCPs and internal teams.
- Escalate issues to Clinical Leads appropriately and promptly.
- Participate in on‑call rotation (approximately 1 weekend per month) and holiday coverage.
- Monitor and improve patient readmission rates and caregiver satisfaction.
- Associate's or Baccalaureate degree in Nursing.
- Current, valid RN license in PA or Compact/Multi‑state unencumbered license.
- 2+ years clinical RN experience.
- 1+ year home health experience.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to work flexible hours, including weekends.
- Strong communication and interpersonal skills.
