UPMC Home Health in State College seeks a dedicated LPN to join the home health care team. You will exercise sound nursing judgment, follow LPN Practice Guidelines, and collaborate with RNs and other providers to plan, implement, and evaluate patient care in the home setting.
Responsibilities include in-field nursing assessments, patient teaching, and coordination with the health care team. The schedule is Monday–Friday with weekend coverage rotations; benefits and a sign-on bonus are available
