This individual must be willing to assist with nursing functions for individuals with disabilities, co-existing medical and mental health conditions. This role will help ensure the individuals’ health, safety, and welfare, while building relationships and encouraging community inclusion allowing them to experience life to its fullest. This position requires an individual who is flexible, a team player, takes direction, highly organized and efficient.

• Completes health appraisal when consumer is entered into the program
• Provides assistance for interactions with appropriate medical professionals as necessary
• Attends all discharge meetings from hospitals, rehabilitation centers, etc. for planning and follow-up, with Program Specialist
• May be required to provide direct care and complete work shifts at the nursing pay rate based on program and consumer needs
• As necessary, assists consumer and staff with any treatments prescribed by physician or any other medical professional
• Evaluates, observes, and assesses physical health conditions and when necessary, documents information
• Performs monthly routine non-evasive health examinations
• Trains staff to correctly follow through with physicians' orders
• Assist in providing training or developing training programs for all staff of individuals when necessary
• Obtain Medication Administration Trainer Certification per Office of Developmental Programs Train the Trainer course in order to train staff in Medication Administration Procedures. Assist in training staff as needed
• Assists in administering, or monitoring administration of, medications. Follows ODP guidelines for correct Medication Administration Procedures
• Monitor consumers’ medication administration as needed, to ensure medication is being administered as prescribed
• Complete a monthly medication review checklist for all  consumers and assists with MH program consumers as needed
• Completes and facilitates accurate medication disposal procedures
• Assist with requesting medication refills, weekly and as needed pill counts, and discussing medication with medical professionals and pharmacists
• Coordinate arrangements for medical equipment as necessary and/or as prescribed
• Monitors vitals
• Monitors intakes and discharges when necessary
• Educates consumers on medication, physical health, psychiatric health, nutrition, and self-care
• Educates staff on any needed physical health matter for consistency in care
• Works with the  team to complete the consumer’s Health Risk Screening Tool (HRST) clinical ratings and responds to reviews
• Continually coordinates care with pharmacy, physicians, and other health care providers
• Participates in treatment team and ISP meetings as necessary
• Provides medication/health teaching
• Promotes healthy practice in all aspects of conduct of life
• Interacts with families, medical providers, other agencies, and any other designated personnel in a positive and professional manner
• Assures the protection of all individuals from abuse and neglect; provides an environment that promotes general health and when applicable, healing from injury or illness; obtains assistance from medical personnel as needed; documents results of these individual contacts and notifies appropriate personnel as applicable
