Job Summary
Our client is seeking a Nurse Home Visitor for their Nurse Family Partnership Program. This role involves active participation in fieldwork, providing superior home visiting services, and building strong client relationships in support of maternity and women's health. The position starts as soon as possible with a 35-hour week schedule typically covering Monday through Friday from 9 AM to 5 PM.
Key Responsibilities
- Provide home visiting services to clients.
- Build and maintain a caseload, scheduling a maximum of 12-15 visits per week.
- Participate in 1-hour supervision calls and team meetings every Wednesday from 10 AM to 12 PM.
- Engage in shadow visits to understand the model better and hold client events.
- Utilize telehealth as a supplement to in-home visits.
- Associates degree required; BSN preferred.
- Current New York State RN license.
- Current CPR certification.
- Bilingual in English and Spanish required.
- Open to new graduates, though experience in women's health, maternity care, public health, or home visiting is preferred.
- Fieldwork or home care experience is essential.
- High work ethic and time management skills are critical.
- Maternity or labor delivery background is advantageous.
- Must have reliable transportation and a valid driver's license for occasional travel, including quarterly trips to Tribeca to pick up equipment.

Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)).
- 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1/1, 4/1, 7/1, and 10/1).
