Licensed Practical Nurse

VNS Health is seeking a dedicated Licensed Practical Nurse to deliver compassionate home and community-based care throughout New York City. The LPN will provide direct patient care, administer medications, monitor health status, and support individualized care plans under the supervision of an RN. In this role, you'll educate patients and families, document care accurately, and collaborate with an interdisciplinary team. Responsibilities

• Provide compassionate nursing care to patients in their homes and community settings
• Administer medications and treatments as prescribed
• Monitor and document vital signs, patient status, and responses to care
• Collaborate with RNs, physicians, therapists, and caregivers to implement care plans
• Educate patients and families on disease management, medications, and home care techniques
• Ensure compliance with infection control and safety protocols
• Accurately maintain electronic health records and required documentation
• Communicate changes in patient condition to the care team promptly
