Centers Adult Day Care seeks a Licensed Social Worker to support seniors and adults with disabilities in our adult day health program. The Social Worker will conduct psychosocial assessments, develop and update person-centered care plans, coordinate services with families, healthcare providers, and community agencies, and advocate for participant needs. Responsibilities include counseling, crisis intervention, care transitions, and leading support groups. The role also supports intake, documentation, and compliance with state regulations while contributing to a compassionate, team-based environment focused on safety, dignity, and independence.
Responsibilities
- Conduct psychosocial assessments for seniors and adults with disabilities
- Develop, implement, and update person-centered care plans
- Provide individual and group counseling and short-term interventions
- Coordinate services with families, healthcare providers, and community agencies
- Support participant intake, eligibility, and transition planning
- Advocate for participant rights, safety, dignity, and independence
- Maintain timely, accurate documentation and care records
- Ensure compliance with state regulations and agency policies
- Lead or support caregiver and participant support groups
- Collaborate with interdisciplinary team on case reviews and programming
