We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing team. As an LPN at Browns Health and Rehabilitation Center, you will play a vital role in providing high-quality clinical care, supporting the health and well-being of our residents, and collaborating with an interdisciplinary team to ensure the best outcomes.

Responsibilities and Qualifications:

• Provide direct nursing care and assist in the delivery of resident-centered services.

• Administer medications and treatments in accordance with physician orders and facility policies.

• Monitor resident conditions, recognize changes in status, and communicate updates to the care team.

• Assist in developing and updating care plans while maintaining accurate and timely documentation.

• Collaborate with physicians, nurses, therapists, and other staff to ensure coordinated care.

• Maintain compliance with all facility policies, procedures, and regulatory standards.

• Hold an active, unencumbered LPN license in the state, or a valid multistate license if applicable.

• Demonstrate strong clinical skills, critical thinking, and attention to detail.

• Communicate effectively and professionally with residents, families, and team members.

• Prior experience in long-term care or skilled nursing preferred but not required.
