Independent Contractor-Family Support Services

Health Connect America, Inc. seeks an Independent Contractor - Family Support Services professional to provide in home and community based support to children, adolescents, and families. Responsibilities include implementing family support plans, teaching parenting and coping skills, coordinating with clinicians and case managers, documenting visits, and linking families to community resources. Contractors collaborate within a multidisciplinary team, maintain professional boundaries, uphold confidentiality, and model positive behavior while supporting client goals.

Responsibilities

• Provide in home and community-based family support services to children, adolescents, and families.
• Implement and follow individualized family support or service plans developed by clinicians or case managers.
• Teach and model parenting, communication, daily living, and coping skills.
• Coordinate with therapists, case managers, schools, and community partners to support client goals.
• Document visits, progress notes, and client contacts accurately and timely in required systems.
• Connect families to community resources such as housing, food assistance, education, and healthcare.
• Monitor family safety and stability, reporting concerns according to policy and regulation.
• Use trauma-informed, client-centered approaches that support dignity, autonomy, and engagement.
• Maintain professional boundaries, confidentiality, and compliance with ethical and legal standards.
• Participate in supervision, team meetings, and ongoing training as required by the contract.
