In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem - Staunton, VA

In-Home Clinical Evaluation

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• The visit lasts about 45 minutes and includes medication and medical history review, physical examination and diagnostic tests (e.g., diabetic eye exams, spirometry, etc.) as ordered by the health plan.
• Clinicians use an iPad and an intuitive clinical workflow; no prescriptive authority or lab ordering required.

• Build trusted relationships with patients and help improve health outcomes.
• Deliver care nationwide, connecting patients to the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the required duties.
• Travel locally and across licensed states as required.
• Conduct virtual visits when appropriate.
• Provide Diagnostic & Preventive Services products such as spirometry, DEE, etc.

A Note on Our Work Environment:
The primary workplace is the private residence of health plan members. Clinicians must be able to safely perform duties in homes that may contain pets, secondhand smoke, and allergens.

• Hourly base rate plus incentive structure (e.g., per visit completed).
• Malpractice insurance coverage during Signify Health‑related activities.
• Supplies and other perks.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Able to cover primary locations:
  Staunton, VA.

Pay Range: $41.85 - $90.13 per hour
