In-Home Nurse Practitioner or Physician Assistant - Staunton, VA

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow and are not required to prescribe medicine or order lab tests.

• Help visit millions of people nationwide with the goal of connecting them to the right care for their unique needs
• Work with Signify Health as a clinician colleague, fulfilling the responsibilities of the specified role
• Travel locally and within licensed states as needed, and conduct virtual visits when required
• Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, DEE, bone density screening, etc.

As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Pay range: $87,035.00 – $. The range represents a base hourly rate or a base annual full‑time salary; the actual offer will depend on experience, education, geography and other factors.
