Job Description

HomeMD Housecall Services serves the geriatric population in Michigan. We are a professional, supportive, and growing nurse‑owned business. Responsibilities
  • Conduct comprehensive physical examinations and health assessments of patients
  • Diagnose and treat acute and chronic illnesses, injuries, and conditions
  • Order, perform, and interpret diagnostic tests and procedures
  • Prescribe medications and treatments, including controlled substances, as appropriate
  • Develop and implement patient care plans in collaboration with the healthcare team
  • Provide patient education on disease prevention, management, and treatment options
  • Monitor and evaluate patient responses to treatments and adjust care plans as needed
  • Collaborate with other healthcare professionals to coordinate patient care
  • Maintain accurate and up-to-date medical records
Requirements
  • Valid Nurse Practitioner license in Michigan
  • Master's degree in Nursing from an accredited program
  • Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Adult‑Gerontology, Pediatrics)
  • Minimum of 1 year of experience as a Nurse Practitioner in a clinical setting
  • Proficiency in HCC, ICD‑10 coding, and medical terminology
  • Experience working in various healthcare settings such as nursing homes, assisted living, memory care, and individual homes
  • Ability to perform basic medical procedures
  • Knowledge of infection control protocols and practices
  • Strong communication skills with the ability to effectively educate patients, caregivers, and families on health‑related topics
