Job Overview

Strategic Staffing Solutions (S3) is hiring a Case Manager RN for a large client in Detroit, MI. This role is fully remote and requires a current, active, unrestricted Michigan Registered Nurse license.

• Job Title: Case Manager RN
• Duration: 6 months (1-year contract with opportunity for extension and conversion)
• Location: Detroit, MI (Remote within Michigan)
• Compensation: W2 rate: $38 hourly; no paid days off or holidays. Medical coverage available.

As a Case Manager RN, you will coordinate a multidisciplinary team to deliver a holistic, person‑centric care management program for a diverse health plan population.

• Lead coordination of a regionally aligned multidisciplinary team including medical and behavioral health professionals.
• Use the case‑management process to assess, develop, implement, monitor, and evaluate care plans.
• Assess members’ health, psychosocial needs, cultural preferences, and support systems.
• Engage members/caregivers to develop individualized plans, address barriers, identify care gaps, and promote improved outcomes.
• Arrange resources such as community services, mental health, substance abuse, financial support, and disease‑specific services.
• Coordinate care delivery among providers, community agencies, and families.
• Advocate for members and promote self‑advocacy.
• Deliver education on health literacy, medication, nutrition, and self‑management.
• Monitor and evaluate care‑plan effectiveness and adjust as needed.
• Accurately document interactions.
• Prepare members/caregivers for discharge or transfer to support continuity of care.
• Secure durable medical equipment and transportation services and communicate to relevant parties.
• Adhere to professional standards, protocols, and quality & production goals.
• Pursue professional development and maintain Certified Case Manager (CCM) certification.

• Nursing Diploma or Associate’s degree required.
• Bachelor’s degree in nursing strongly preferred.
• Minimum 3 years of clinical nursing experience in acute/post‑acute and community settings.
• Preferred: 1 year of case‑management experience in managed care.
• Experience managing patients telephonically and via digital channels preferred.

• Current, active, unrestricted Michigan RN license required.
• Certified Case Manager (CCM) required or to be obtained within 18 months of hire.
• Certified Chronic Care Professional (CCP) preferred.

• Empathy, supportive listening, and motivational interviewing skills.
• Time management and organizational skills.
• Intermediate computer proficiency with Microsoft Office (Excel, One Note, Outlook, Teams, Word).
• Teamwork ability while also working independently.
• Excellent interpersonal and written & verbal communication skills.

• Pediatrics, SNF/SAR/LTAC, hospital or other facility case management, home health care, ER, ICU/Step‑down, utilization management.
