POSITION SUMMARY

Provides direct behavioral health and case management interventions to individuals involved in the criminal justice system, assisting individuals to gain needed resources and services and facilitating their engagement in treatment. The position also provides outpatient competency education to individuals referred by the Colorado Office of Civil and Forensic Mental Health.

This position will cover Sterling, CO, Fort Morgan, CO, and any additional locations as needed within the Center's service area.

Responsibilities
  • Provide supportive behavioral health interventions to clients either in individual or group settings, including MRT Groups.
  • Complete Brief Screening of all clients referred through a judicial entity (Probation, Parole, Court, Restoration Program, Youth Diversion and DUI) and engage client in case management services as appropriate.
  • Develop, monitor, and follow up on client case management service plans.
  • Assist clients with resource acquisition, linkage to agency and community services, behavioral health skill development.
  • Working collaboratively with the treatment provider(s), coordinate (be knowledgeable about) services provided to judicially referred clients. Examples include: communicating weekly with therapist(s) on all new judicially-referred admissions; making sure that CM services are included in the Treatment Plan for all clients served,; attend clinical team or ACT meetings; confirm client attendance and UA results with clinician of record, work with clinician to enhance treatment engagement and success, etc.

  • Serve as primary source of contact and information for referral parties, including local probation departments, courts and others; attend probation meetings regularly.
  • If assigned, represent the agency in local problem-solving courts.
  • Provide services in a variety of settings, including client homes, community locations and Centennial offices.
  • Maintain accurate and timely clinical records consistent with Center standards.
  • Participate in efforts to collaborate with local jails to facilitate care coordination for individuals releasing from incarceration into Centennial communities.
  • Serve as a Case Management resource to individuals releasing from local jails, assisting with engagement into treatment and other practical needs during the transition phase.
  • If assigned, facilitate the DRS process for DUI clients by entering TMS data regularly and providing the completed DRS form to clients when treatment is successfully completed.
  • Respond to referrals for Outpatient Competency Restoration services in a timely manner.
  • Provide outpatient Restoration services to referred clients as directed by Office of Civil and Forensic Mental Health, and complete all required documentation and reporting relating to these services if assigned.
  • Participate in meetings, training and in services.
  • Participate in supervision sessions.
  • Perform other job duties as assigned.
  • Demonstrate a commitment to continuous skill development to competently serve individuals from diverse cultures and backgrounds.
Education

EDUCATION

Bachelor’s degree in psychology, social work, or other human services field required

Experience working in behavioral health, human services, adult education, rehabilitation preferred but not required
