Talin Home Healthcare LLC is looking for a motivated and detail-oriented Home Health Intake Coordinator to join our growing team in Sterling, VA. In this vital role, you'll be the first point of contact for patients and families, helping to connect them with the compassionate care they need. Responsibilities- Manage the end-to-end patient intake process, including receiving and processing referrals from hospitals, physicians, and discharge planners
- Verify patient insurance eligibility and obtain authorizations for home health services
- Coordinate with clinical staff to ensure timely start-of-care for new patients
- Maintain accurate and up-to-date patient records in the EMR/EHR system
- Communicate effectively with patients, families, referral sources, and internal care teams
- Ensure compliance with Medicare, Medicaid, and private insurance guidelines for home health admissions
- Track and follow up on pending referrals and authorizations to minimize delays in care
- Prior experience in home health intake, medical scheduling, or healthcare coordination strongly preferred
- Familiarity with Medicare/Medicaid home health regulations and insurance authorization processes
- Proficiency with EMR/EHR software and Microsoft Office Suite
- Strong organizational skills with the ability to manage multiple referrals simultaneously
- Excellent verbal and written communication skills
- Compassionate, patient-focused demeanor with a commitment to quality care
- Twice a month will need to make trips to other doctor's offices to pick up signed POC documents (mileage is paid)
- High school diploma or equivalent required;
Associate's degree in healthcare administration or related field preferred - Required to work onsite in the Office in Sterling 30 to 40 hours a week 5 days a week
