• Monitor patient progress and communicate changes to the physician and care team.
• Educate patients and families on disease processes, medication management, and self-care techniques.
• Document all visits and interventions accurately and timely in the electronic medical record (EMR).
• Ensure compliance with all applicable federal, state, and local regulations and company policies.
