Your APP – Nurse Practitioner Responsibilities
  • Support in‑home, patient‑facing visits to address patient care needs
  • Perform primary care, initial screenings, exams, and documentation
  • Partner with clinicians in the Care Community to manage patient health
  • Engage attributed patients into the Well Be program
  • Facilitate decision‑making by providing educational resources
  • Address urgent care needs and transition of care visits
  • Collaborate with community team to complete full patient assessments
  • Order, interpret, and analyze diagnostic tests
  • Prescribe and manage medications and treatments under state supervisory agreement
  • Update patient records and check accuracy at each visit
  • Deliver care and complete regular patient reviews with supervising physician
  • Support clinical documentation, HEDIS measures, and engagement of patients
  • Utilize available resources and refer to internal/external supports
  • Collaborate with interdisciplinary team and participate in team meetings
  • Perform other tasks to accomplish team objectives
APP – Nurse Practitioner Qualifications
  • Master’s Degree in Nursing with focus on Family, Adult, Geriatric, or Adult‑Geriatric NP (required)
  • Active, unencumbered NP license in Wisconsin and residence in the state (required)
  • Current board certification (ANCC or AANP) (required)
  • BLS certified and active NPI# as NP (required)
  • CPR certification (required)
  • Active or eligible DEA licensure (required)
  • Valid driver’s license, car insurance, and willingness to drive to patient homes (required)
  • Proof of active auto insurance policy (required)
APP – Nurse Practitioner Skills We Value
  • 3+ years NP experience (preferred); new graduates considered
  • Prior healthcare experience in medical assistant, CNA, EMT, lab assistant, RN, or similar (preferred)
  • Experience in value‑based care, home care, primary care, hospice, palliative care, or geriatrics (preferred)
  • Experience with telehealth care delivery (preferred)
  • Home visits required in various residential settings
  • Proficient in Microsoft Office and EHR, scheduling, and Ring Central applications
Supervisory Responsibility

This position will have supervisory responsibility.

Travel Requirements

Travel may be required up to 100% locally to deliver patient care.

Work Conditions

Ability to lift up to 30 lbs.; patient transport may involve lifts up to 50 lbs. and assisting with weights over 100 lbs. Ability to stand for extended periods, drive to patient locations (home, hospital, SNF, etc.), and maintain fine motor skills and visual acuity.
