Are you a compassionate Registered Nurse who is passionate about leading clinical teams and making a meaningful impact in the lives of seniors? Health Dimensions Group is seeking a Director of Wellness (RN) to lead the clinical operations at Dimensions Living Stevens Point
, our 75 RCAC, 16 CBRF, and 56 IL-bed managed community in Stevens Point, Wisconsin.

What You'll Do (AKA: Your Superpowers)
Lead Clinical & Resident Wellness
  • Oversee all clinical and wellness services throughout the community.
  • Ensure residents receive exceptional, person-centered care through comprehensive assessments, individualized care planning, and medication management.
  • Promote a culture focused on dignity, independence, safety, and quality of life.
  • Partner with residents, families, physicians, pharmacies, and outside providers to coordinate exceptional care.
Develop & Inspire Your Team
  • Recruit, mentor, and develop nurses and caregivers to build a high-performing wellness team.
  • Foster a positive, collaborative culture focused on accountability, engagement, and professional growth.
  • Provide coaching, education, and ongoing support to strengthen clinical performance and resident outcomes.
Drive Clinical Excellence & Compliance
  • Maintain survey readiness and ensure compliance with all state and federal regulations.
  • Monitor clinical documentation, resident assessments, incident reporting, infection prevention, and medication management.
  • Lead quality improvement initiatives while partnering with pharmacy consultants, physicians, and interdisciplinary teams.
Partner Across the Community
  • Collaborate closely with the Executive Director and department leaders to support community goals.
  • Participate in resident care conferences, family meetings, and interdisciplinary planning.
  • Help cultivate a community where wellness is integrated into every resident experience.
Support Residents & Families
  • Serve as a trusted clinical resource for residents and their families.
  • Build strong relationships through proactive communication, education, and compassionate support.
  • Promote programs that enhance residents' physical, emotional, and social well-being.
What You Bring to the Table (Besides Your Clinical Expertise)
Education & Licensure
  • Graduate of an accredited school of nursing.
  • Current Registered Nurse (RN) license in the State of Wisconsin.
  • CPR certification preferred.
  • Strong understanding of infection prevention and senior wellness best practices.
Leadership & Clinical Experience
  • Minimum two years of nursing leadership experience within senior living.
  • Experience leading, coaching, and developing clinical teams.
  • Demonstrated ability to manage resident care, regulatory compliance, and quality initiatives.
Clinical Knowledge & Operational Skills
  • Working knowledge of Wisconsin assisted living regulations (RCAC/CBRF or comparable senior living regulations).
  • Strong assessment, care planning, and clinical decision-making skills.
  • Excellent organizational skills with the ability to prioritize multiple responsibilities.
  • Comfortable partnering with interdisciplinary teams and outside healthcare providers.

* Wage range posted includes base salary with an additional 10% annual bonus based on Key Performance Indicators.
