Community Health Worker
Washington County is currently hiring for a Community Health Worker. The Community Health Worker assists individuals in accessing and navigating health and social services, with a focus on all aspects of health and wellness for an individual, family and the community. They will serve as a liaison between county services and the individual/community to simplify the process to access services, improve the quality and cultural competence of service delivery while building community capacity.
Bicultural and/or bilingual candidates with close ties to cultural communities are encouraged to apply. Current need is for candidates with a close connection to the Spanish Speaking Community. DESIRABLE
EXPERIENCE:
Experience as a Community Health Worker Experience (paid, unpaid or volunteer) with community engagement, providing culturally appropriate resources information, participating in community projects, and/or coordination of care to individuals and groups within diverse communities. This includes face-to-face, phone or group interactions, and may include traveling to an individual's home, community locations, various agencies, and other outreach destinations Experience (paid, unpaid or volunteer) building relationships in a community-based setting with local communities and/or community-based organizations (such as schools, community, faith-based organizations, and health care facilities) Experience providing services to individuals from a variety of cultural and ethnic backgrounds SELECTION TECHNIQUE:
Your training and experience will be rated based on the required application material submitted and the top-scoring candidates will be forwarded to the hiring department for further consideration. HOURS / DAYS: 8:00 am - 4:30 pm / Monday - Friday plus overtime hours as needed. Work schedules (hours and days) are subject to change based on department needs. ANTICIPATED PAY RATE: $28.62 - $39.00 Hourly E-Verify Participation:
Washington County participates in the federal E-Verify program. This means that Washington County will provide the Social Security Administration (SSA) and, if necessary, the Department of Homeland Security (DHS), with information from each new employee's Form I-9 to confirm work authorization.

MINIMUM QUALIFICATIONS:
- High school diploma or GED with at least 3 years of experience (combination of paid and unpaid) in human services or related field.
- or completion of at least two years of post-secondary education with at least two years of experience (combination of paid and unpaid) in a human services or related field.
- or completion of Community Health Worker certificate through an approved college or technical school and two years of experience (combination of paid and unpaid) in a human services or related field.
- Must have and maintain a valid class D driver's license.
