Job Title: RN Visit Nurse – Home Health, 1:1 Patient Care + $4,000 Sign-On Bonus
Location: Greater Twin Cities Metro Area and surrounding Minnesota service area
Schedule: Full-Time with On-Call and Weekend Rotation
Compensation: $75,000 – $100,000 per year plus $4,000 Sign-On Bonus

As a RN Visit Nurse, you will provide true one-on-one, in-home patient care
, building relationships and delivering meaningful outcomes in a setting that allows you to slow down and focus.
- 1:1 patient care in homes and community settings.
- Full-time stability with consistent scheduling.
- Autonomy in the field with strong clinical support.

- Deliver treatments including wound care, medication management, and assessments
. - Coordinate care with physicians, therapists, and interdisciplinary teams.
- Educate patients and families on care plans, medications, and recovery goals
. - Maintain accurate, timely documentation in the EMR.
- Experience in home health, med-surg, TCU, rehab, or clinical nursing
. - Strong clinical judgment and ability to work independently.
- Excellent communication and patient education skills.
- Valid driver’s license and reliable transportation.
