$75,000 – $100,000 per year plus $4,000 Sign‑On Bonus

As a RN Visit Nurse, you will provide true one‑on‑one, in‑home patient care, building relationships and delivering meaningful outcomes in a setting that allows you to slow down and focus.

• 1:1 patient care in homes and community settings.
• Full‑time stability with consistent scheduling.

• Provide skilled nursing visits in patient homes and care settings.
• Deliver treatments including wound care, medication management, and assessments.
• Coordinate care with physicians, therapists, and interdisciplinary teams.
• Educate patients and families on care plans, medications, and recovery goals.
• Maintain accurate, timely documentation in the EMR.

• Active Minnesota Registered Nurse (RN) license.
• Experience in home health, med‑surg, TCU, rehab, or clinical nursing.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.
• Valid driver’s license and reliable transportation.
