Join our team as a day shift, full‑time, Population Health LPN (Licensed Practical Nurse) Patient Care Coordinator in Tulsa, OK.
Full responsibility for outreach to patients outside of face‑to‑face office visits to help them understand and manage their health, navigate the health system, and meet their health goals.
Utilize the practical nursing process, providing focused assessment of acute and chronic conditions, disease‑management education, and care coordination across the healthcare continuum.
Job Requirements- Graduate of an accredited school of nursing
- Possession of a valid State LPN license
- BLS certification obtained within 14 days of hire or transfer into the role and prior to providing direct patient care
Job Requirements
- A minimum of two years of professional nursing experience in a clinic setting
