** is responsible for assessing patient needs, developing individualized care plans, coordinating services, and providing skilled nursing care to patients recovering from illness, injury, surgery, or managing chronic conditions.

The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for helping patients maintain independence while achieving their healthcare goals. This position requires collaboration with physicians, therapists, caregivers, and interdisciplinary team members to ensure optimal patient outcomes.

Qualifications:

* Current, unrestricted Registered Nurse (RN) license.
* Graduate of an accredited nursing program.
* Minimum one year of nursing experience required; home health experience preferred.
* Knowledge of Medicare, home health regulations, and clinical documentation standards.
* Strong assessment, clinical decision-making, and organizational skills.
* Excellent communication and patient education abilities.
* Valid driver's license, reliable transportation, and proof of auto insurance.###

Key Responsibilities:

* Conduct comprehensive patient assessments and develop individualized plans of care.
* Provide skilled nursing services, including wound care, medication management, IV therapy, and disease management education.
* Monitor patient progress and adjust care plans as needed.
* Educate patients and family members on treatment plans, medications, and disease processes.
* Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
* Maintain accurate, timely, and compliant clinical documentation.
* Identify changes in patient condition and communicate appropriately with the healthcare team.
* Participate in case conferences and interdisciplinary team meetings.
* Ensure compliance with agency policies, state regulations, and Medicare guidelines.
