The Hospice RN Case Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other disciplinary team members while maintaining standards of professional nursing and clinical competency.
- Valid license as a registered nurse in the state of Oklahoma.
- Home health and/or Hospice experience is highly desired.
- Current BLS certification within 30 days.
- Minimum one-year of recent professional nursing experience.
- Having and maintaining a valid Oklahoma driver's license and satisfactory driving record as a condition of employment.
- Able to cope with emotional stress and be tolerant of individuals lifestyles.
- Sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death.
- Basic computer skills required.
Physical Requirements:
- Must have adequate perception of sounds or adequate hearing with corrections.
- Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials.
- Able to speak clearly and distinctly with staff, physicians, patients, and patients' families.
- Must have strength and stamina for prolonged walking, standing, and dealing with stressful situations.
- Able to hear, or correctable with hearing devices, and able to talk to converse with others in person and over the phone.
- Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials.
- Able to frequently lift at least 60 lbs., reach, pull, push, bend, stoop, and kneel.
- Possess fine motor skills and hand/eye coordination to give care (i.e.: insert catheters, start IVs, administer medications, and handle instruments).
- May be exposed to virus, disease, and infection from patients, co-workers, family member and others as well as specimens in working environment.
- May experience traumatic situations in the family setting.
