Responsibilities:- Provide direct patient care and nursing services across various healthcare homes and client locations
- Administer medications, treatments, and procedures in accordance with physician orders and care plans
- Monitor and document patient vital signs, symptoms, and progress in medical records
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care
- Educate patients and families on health management, medications, and post-care instructions
- Adapt quickly to new clinical environments, policies, and patient populations
- Maintain compliance with state, federal, and facility-specific healthcare regulations
- Active Licensed Practical Nurse (LPN) license in good standing in the state of Georgia
- Previous clinical experience in a hospital, long-term care, or home health setting preferred
- Willingness and ability to travel to multiple homes or patient locations as required
- Strong clinical assessment, documentation, and patient communication skills
- BLS/CPR certification required.
- Reliable, self-motivated, and able to work independently with minimal supervision
- Flexibility to work on-call as needed
