The United We Lead Foundation (UWLF) is seeking a compassionate,motivated, and community‑focused Part‑Time Bilingual Community Health Worker (CHW) to join our team in Stockton, California. This position plays a vital role in improving accessto health education, community resources, and supportive services for underservedfamilies, including migrant, immigrant, and English Learner communities.
The Community Health Worker will build relationships with community members, conductoutreach, provide health education, connect families with resources, and support the implementation of health and wellness initiatives throughout San Joaquin County and surrounding communities.
This is an in‑person position based out of the UWLF Stockton office with local travel required for community outreach, workshops, and events.
- Provide community-based health education and outreach, promoting wellness and available community resources.
- Serve as a liaison between community members and healthcare providers, helpingfamilies access medical, behavioral health, and social services.
- Assist with community outreach events, health fairs, workshops, and wellnessinitiatives.
- Support clients in navigating healthcare systems, including schedulingappointments, understanding insurance options, and connecting to local resources.
- Conduct needs assessments and provide individualized support and referrals.
- Maintain accurate case notes, outreach logs, and required program documentationwhile ensuring confidentiality.
- Collaborate with healthcare providers, schools, nonprofit organizations, and community partners to improve service delivery.
- Assist in developing and distributing bilingual educational materials.
- Promote health literacy through education on nutrition, preventive care, chronic disease prevention, mental health, and healthy lifestyles.
- Build trusting relationships within the community to increase participation in healthand wellness programs.
- Perform other duties as assigned in support of UWLF's mission.
- High School Diploma or equivalent required.
- Associate's degree or coursework in Community Health, Public Health, Human Services, Social Work, or a related field is preferred.
- Fluent in both English and Spanish (written and verbal).
- Community Health Worker (CHW) Certification preferred, or a minimum of 2,000hours of CHW experience or three (3) years of related paid or volunteerexperience.
- Experience working with underserved, immigrant, migrant, or culturally diversepopulations preferred.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently while collaborating effectively with a team.
- Compassionate, patient, and culturally responsive approach to serving families.
- Knowledge of healthcare systems, community resources, and social services in San Joaquin County is preferred.
- Proficiency with Microsoft Office and Google Workspace preferred.

This is a part-time, in-person position based at:
United We Lead Foundation 5637 N. Pershing Ave., Suite F18 Stockton, CA. - Typical schedule is 20–29 hours per week, with occasional evening or weekend hours required to support community outreach events and workshops.
- Compensation $18.00–$25.00 per hour, depending on education, certifications, and related experience.
