About the Role:
As a Home Health Manager, you'll lead a high-performing team delivering exceptional care to participants in their homes and within nursing or board and care facilities. This role is pivotal in ensuring seamless, personalized, and high-quality care for a frail and elderly population.

Key Responsibilities:
  • Direct daily operations of the home care department, ensuring all workflows and policies are followed.
  • Hire, train, mentor, and evaluate home care team members to ensure optimal performance.
  • Oversee nursing assessments and interdisciplinary team (IDT) participation to evaluate care effectiveness.
  • Ensure documentation and delivery of nursing interventions align with participants' care plans.
  • Supervise the quality of care delivered in external nursing and board and care homes.
What Youll Bring:
  • Bachelors Degree preferred (or equivalent experience).
  • At least 2 years of nursing experience in home health, preferably with frail or elderly populations.
  • Valid California RN License.
  • Deep understanding of nursing care for medically complex and elderly patients.
