The Home Health Registered Nurse (RN) plays a critical role in delivering personalized nursing care to patients within their homes, ensuring their health and well‑being outside of traditional clinical settings. This position involves comprehensive assessment, planning, implementation, and evaluation of patient care plans tailored to individual needs, often for those with chronic illnesses, post‑surgical recovery, or disabilities. The Home Health RN acts as a vital liaison between patients, families, physicians, and other healthcare professionals to coordinate care and promote optimal health outcomes.

By providing education, support, and clinical expertise, the nurse empowers patients and caregivers to manage health conditions effectively. Ultimately, this role contributes to improving patient quality of life while reducing hospital readmissions and supporting independent living.

• Current and valid Registered Nurse (RN) license in the state of practice within the United States.
• Minimum of an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution.
• Basic Life Support (BLS) certification.
• Proven clinical nursing experience, preferably with at least one year in home health, community health, or related nursing practice.
• Strong knowledge of nursing principles, patient care standards, and home health regulations.

• Bachelor of Science in Nursing (BSN) degree.
• Certification in Home Health Nursing (e.g., Certified Home Health Nurse - CHHN).
• Experience with electronic health records (EHR) and telehealth technologies.
• Advanced certifications such as Wound Care, IV Therapy, or Chronic Disease Management.
• Demonstrated experience in patient education and care coordination.

• Conduct thorough patient assessments to evaluate health status, needs, and progress in the home environment.
• Develop, implement, and update individualized nursing care plans based on patient assessments and physician orders.
• Administer medications, treatments, and therapies as prescribed, ensuring patient safety and adherence to protocols.
• Educate patients and their families on disease management, medication administration, wound care, and preventive health measures.
• Collaborate with interdisciplinary healthcare teams to coordinate care and communicate patient status and changes.
• Document all nursing care, patient responses, and clinical observations accurately and timely in compliance with regulatory standards.
• Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
• Provide emotional support and counseling to patients and families to help them cope with health challenges.

The Home Health RN utilizes clinical assessment skills daily to evaluate patient conditions accurately and develop effective care plans. Strong communication skills are essential for educating patients and families, as well as collaborating with healthcare teams to ensure coordinated care delivery. Time management and organizational skills enable the nurse to manage multiple patient visits efficiently while maintaining thorough documentation. Critical thinking and problem‑solving abilities are applied to adapt care plans based on patient progress and emerging health issues.

Additionally, proficiency with technology, including electronic health records and telehealth platforms, supports accurate record‑keeping and remote patient monitoring.
