Job SummaryWe’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.
Responsibilities- Provide nursing care according to the RN‑developed Plan of Care.
- Monitor and document changes in patient condition; report to RN/physician as needed.
- Assist with activities of daily living to promote patient independence.
- Educate patients and caregivers within LPN/LVN scope of practice.
- Maintain accurate EMR documentation.
- Participate in case conferences and coordinate care with the home health team.
- Follow infection‑control, HIPAA, and company policies.
- Current LPN/LVN license in the state of practice.
- Graduate of an accredited practical/vocational nursing program.
- Minimum1 year acute care experience preferred.
- Current CPR/BLS certification.
- Valid driver’s license, reliable insured vehicle.
- Strong communication, clinical judgment, and ability to work independently.
- Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
- Licensed or registered to practice as a PT in the state of practice.
- Preferably at least two (2) years of experience as a PT.
- Possesses and maintains good physical stamina and mental health. Presents a pre‑employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
- Has the ability to read and follow written instructions and document care given.
- Is self‑directing and has the ability to work with little direct supervision.
- Possesses and maintains current CPR certification.
- Is flexible and cooperative in fulfilling the role obligations.
- Demonstrates tact, patience, and good personal hygiene.
- Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
- Must be able to verbally communicate in the language of the client and document all client‑related activity practices.

Pay per visit ranges from $35 to $55 visit commensurate with experience and depending on type of visit.
