Position Overview

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care to frail, elderly, and medically complex patients. The NP will work closely with an interdisciplinary team to manage acute and chronic conditions, coordinate care, and promote quality of life and aging in place.

Responsibilities
  • Provide comprehensive primary care, including assessment, diagnosis, treatment, and preventive care.
  • Manage acute and chronic medical conditions and medications.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Order and interpret labs, EKGs, and other diagnostic tests.
  • Perform appropriate clinical procedures, including injections and immunizations.
  • Coordinate referrals, hospital care, and discharge planning.
  • Participate in interdisciplinary team meetings and care coordination.
  • Educate patients, families, and caregivers on treatment and disease management.
  • Maintain accurate clinical documentation and participate in quality improvement initiatives.
  • Conduct home visits when clinically needed.
Qualifications
  • Graduate of an accredited Nurse Practitioner program.
  • Current California NP license and DEA registration.
  • Current BLS/CPR and First Aid certification.
  • 3-5 years of nursing experience, including at least 1 year with frail/elderly patients.
  • Geriatric, primary care, PACE, community health, or medically complex patient experience preferred.
  • Strong clinical assessment, communication, and care coordination skills.
  • Ability to work effectively within an interdisciplinary team.
