We are seeking a Nurse Practitioner to provide primary care services to a senior population in a collaborative, patient-centered care environment. This role focuses on chronic disease management, care coordination, and helping older adults maintain their health and independence.

Responsibilities:

• Perform initial, annual, and ongoing patient assessments and develop individualized care plans.
• Manage acute and chronic medical conditions for a frail and elderly population.
• Collaborate with physicians, nurses, specialists, and the interdisciplinary care team.
• Provide patient and family education regarding treatment plans and disease management.
• Coordinate specialty referrals, hospital transitions, and follow-up care.
• Maintain accurate and timely clinical documentation.

Qualifications:

• Active California Nurse Practitioner and RN licenses.
• Graduate of an accredited Nurse Practitioner program.
• Minimum of 5 years of nursing experience.
• At least 1 year of experience working with an elderly or frail population.
• Experience in primary care, geriatrics, managed care, skilled nursing, long-term care, or home health preferred.
• DEA registration required within 6 months of hire.
• BLS certification required.
