Job Details
We are seeking a Nurse Practitioner to provide primary care services to a senior population in a collaborative, patient-centered care environment. This role focuses on chronic disease management, care coordination, and helping older adults maintain their health and independence.
Responsibilities- Perform initial, annual, and ongoing patient assessments and develop individualized care plans.
- Manage acute and chronic medical conditions for a frail and elderly population.
- Collaborate with physicians, nurses, specialists, and the interdisciplinary care team.
- Provide patient and family education regarding treatment plans and disease management.
- Coordinate specialty referrals, hospital transitions, and follow-up care.
- Maintain accurate and timely clinical documentation.
- Active California Nurse Practitioner and RN licenses.
- Graduate of an accredited Nurse Practitioner program.
- Minimum of 5 years of nursing experience.
- At least 1 year of experience working with an elderly or frail population.
- Experience in primary care, geriatrics, managed care, skilled nursing, long-term care, or home health preferred.
- DEA registration required within 6 months of hire.
- BLS certification required.
