Description

Position Summary

Under general supervision, this position functions as a direct link between families enrolling in CAPC's trauma-informed, culturally responsive Family Services program and the agency in accordance with the funding contract. The program aims to reduce youth involvement in firearm-related violence, strengthen family protective factors, and promote social-emotional development through mentoring, case management, family support, emotional regulation, violence prevention, and school re-engagement services.

The Family Advocate is responsible for outreach, intake, and enrollment of families, as well as comprehensive assessments of families' strengths, needs, and protective factors. This role provides parent coaching, case management, and connection to community resources to improve family functioning, resilience, and long-term well-being.

The Family Advocate works in close collaboration with the Youth Advocate, ensuring coordinated, trauma-informed, and culturally responsive support for caregivers. Together, they foster trust, strengthen protective factors, and guide families in building skills, navigating challenges, and developing connections that reduce risk and promote positive outcomes for families.

    Specific Duties
  • Outreach & Engagement
    • Conduct proactive outreach to eligible families in alignment with funding contract requirements and program eligibility criteria.
    • Establish trusting, culturally responsive relationships with caregivers using trauma-informed engagement strategies.
    • Present program services clearly to families, ensuring understanding of voluntary participation and program goals related to violence prevention and youth development.
  • Intake & Enrollment
    • Complete intake screenings, eligibility verification, and enrollment documentation in compliance with contract and agency standards.
    • Maintain accurate and timely documentation in required data systems and case records.
  • Comprehensive Assessment
    • Conduct strengths-based, trauma-informed assessments/screenings to identify family needs, protective factors, risk factors, and service priorities.
    • Assess domains including family functioning, social-emotional development, school engagement, community safety, and access to basic needs.
    • Collaboratively develop individualized family service plans that align with program goals of reducing firearm-related violence and strengthening protective factors.
  • Parent Coaching & Skill Development
    • Provide individualized parent coaching focused on emotional regulation, positive discipline strategies, communication skills, and strengthening caregiver-child relationships.
    • Deliver structured, curriculum with families, guiding caregivers through lessons on trauma, communication skills, positive parenting, and family strengthening. Ensure fidelity to the curriculum and integrate lessons into individualized service plans.
    • Support caregivers in developing protective factors such as resilience, social connections, knowledge of parenting and child development, and access to concrete supports.
    • Model and reinforce trauma-informed and culturally responsive parenting approaches.
  • Case Management & Resource Navigation
    • Provide ongoing case management services, including goal tracking, follow-up, and reassessment of needs.
    • Connect families to community-based services such as mental health, substance use treatment, housing, employment, food assistance, and educational resources.
    • Coordinate referrals and warm hand-offs to ensure successful linkage to services.
