Position Overview
Park Springs is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our healthcare team. The NP will provide high-quality care to residents through the Wellness Clinic, conduct home visits, and support members in our Assisted Living facility. This role requires a skilled clinician with the ability to assess, diagnose, and treat a wide range of health conditions while promoting wellness and a holistic approach to care.
LocationPark Springs, Department:
Wellness Clinic
- Provide primary care services including health assessments, screenings, and preventive care
- Develop and implement personalized care plans
- Collaborate with other healthcare providers to ensure comprehensive care
- Conduct home visits to manage chronic conditions and monitor progress
- Educate residents and families on care plans and health practices
- Coordinate follow-up care with the broader healthcare team
- Monitor, assess, and evaluate residents’ overall health status, identifying changes in condition and implementing appropriate clinical interventions
- Prescribe and manage medications and treatments in accordance with established clinical guidelines and regulatory standards
- Work with interdisciplinary healthcare team (physicians, nurses, support staff)
- Maintain accurate electronic health records (EHR)
- Educate residents, families, and staff on wellness and disease prevention
- Ensure compliance with healthcare regulations
- Document all patient interactions and outcomes according to policy
- Master’s Degree in Nursing (MSN)
- Current Nurse Practitioner license in Georgia
- Board certification in Family Medicine, Geriatrics, or related specialty (preferred)
- 2 or more years of clinical experience, preferably in geriatric or longterm care
- Strong assessment, diagnostic, and problem‑solving skills
- Excellent communication and interpersonal abilities
- Familiarity with electronic health record systems (EHR)
- Ability to work independently and in a team environment
- Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds (this includes clinic and beyond visits)
